About The Position

Residential Home Health and Hospice is hiring a Registered Nurse to provide skilled nursing care to patients in their homes in Elgin and surrounding areas. The position offers autonomy, meaningful patient relationships, and opportunities for career growth. It is eligible for a $10,000 first-year success bonus tied to the accomplishment of key achievements, with additional bonus opportunities as employees grow with the organization.

The nurse works collaboratively with an interdisciplinary team while traveling to patient homes within an assigned territory. Field education, mentoring leadership, and a collaborative culture are provided to support success and professional growth.

Requirements

• Active Registered Nurse (RN) license in the state where the employee works.
• At least one year of RN experience and the clinical expertise to deliver quality patient care.
• Valid driver’s license.
• Ability to spend approximately 20% of the workday driving to and from patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate patients and families about disease management and treatment plans.
• Use critical thinking and solve problems in real time.
• Complete thorough, compliant EMR documentation.
• Work independently while collaborating with the care team.
• Travel to patient homes within the assigned territory.
