About The Position

Residential Home Health and Hospice is hiring a Registered Nurse (RN) to provide skilled nursing care to patients in their homes in Elgin and surrounding areas. The role offers autonomy, meaningful patient relationships, and opportunities for career growth. It is eligible for a $10,000 first-year success bonus tied to key achievements, with additional bonus opportunities available as employees grow with the company.

The RN works independently in the field while collaborating with a supportive interdisciplinary team. A dedicated field education team provides support from the first day, and leadership offers mentoring and opportunities for employee feedback to shape improvements.

Requirements

• Active Registered Nurse (RN) license in the state where the employee works.
• At least one year of RN experience and the clinical expertise to deliver quality patient care.
• Valid driver’s license.
• Ability to spend approximately 20% of the workday driving to and from patient locations.

Responsibilities

• Deliver skilled, patient-centered care to patients in a home health setting in collaboration with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate patients and families about disease management and treatment plans.
• Think critically and solve problems in real time.
• Maintain thorough, compliant EMR documentation.
• Travel to patient homes within the assigned territory.
