Home Health Clinical Team Manager (RN)
About The Position
Custom Home Health is hiring a Registered Nurse to manage its home health clinical team. The position is based at the Troy office with a hybrid schedule. The schedule is Monday through Friday, 8:30 a.m. to 5:00 p.m., with weekend backup and a weekly rotation of evening on-call duties (calls only). The role supports quality home care by reviewing referrals, assessing patient needs, assigning clinicians, and helping develop comprehensive care plans. Requirements
- Registered Nurse with current licensure in Michigan.
- At least 2 years of management or supervisory experience in healthcare, preferably in home care.
- Current CPR certification.
Responsibilities
- Receive and review case referrals.
- Assess patient needs and assign appropriate clinicians.
- Provide guidance and support to ensure quality home care services.
- Collaborate with the team to establish therapeutic goals and develop comprehensive care plans.
- Conduct quarterly record reviews and make recommendations for improvement.
- Assist with screening and interviewing new organization personnel and make recommendations for employment.
