About The Position
Custom Home Health is hiring a Registered Nurse to manage its home health clinical team. The position is based at the Troy office with a hybrid schedule. The regular schedule is Monday through Friday, 8:30 a.m. to 5:00 p.m., with weekend backup and a weekly rotation of evening on-call duties by phone. The role involves reviewing referrals, assessing patient needs, assigning clinicians, supporting quality home care, and helping develop and improve care plans and clinical records.
Requirements
- Registered Nurse with current Michigan licensure.
- At least 2 years of management or supervisory experience in healthcare; home care experience is preferred.
- Current CPR certification.

Responsibilities
- Receive and review case referrals.
- Assess patient needs and assign appropriate clinicians.
- Provide guidance and support to ensure quality home care services.
- Collaborate with the team to establish therapeutic goals and develop comprehensive care plans.
- Conduct quarterly record reviews and make recommendations for improvement.
- Assist with screening and interviewing prospective personnel and make employment recommendations.
