About The Position

WelbeHealth PACE helps seniors remain in their homes and communities through comprehensive medical care and community-based services. The Home Care Scheduler plays a critical role in Home Services by coordinating and tracking Home Health and Home Care Assistant assignments to help meet participant needs. Reporting to the Home Care Supervisor, this role provides customer service to participants, families, vendors, and staff and maintains records of scheduled appointments.

Requirements

High school diploma or equivalency.
At least one year of experience as a scheduler in a home health or home care agency.

Nice To Haves

At least one year of experience working with an EMR and scheduling system.

Responsibilities

Coordinate Home Care Assistant and Personal Care Assistant assignments as directed by Home Health RNs and the Home Care Supervisor, including schedule changes and communication with participants, families, and home services staff.
Complete required authorizations in the electronic medical records (EMR) system for approved personal care and home health services.
Ensure medical records for clinicians, home care aides, vendors, and in-home staff visits are received and uploaded into the electronic health record (EHR).
Implement and improve procedures, such as reminder calls, clarifying instructions, and sending necessary materials, to support successful experiences for providers, staff, and participants.
Maintain current and accurate records of scheduled home health appointments and home and personal care assignments, including scheduled times and outcomes.
