About The Position

WelbeHealth PACE helps seniors remain in their homes and communities by providing comprehensive medical care and community-based services. The Home Care Scheduler plays a critical role in Home Services by coordinating and tracking Home Health and Home Care Assistant assignments, helping ensure participants’ needs are met with a high level of care. Reporting to the Home Care Supervisor, the Scheduler provides customer service to participants, families, vendors, and staff and maintains records of scheduled appointments.

Requirements

• High school diploma or equivalency.
• At least one year of experience as a scheduler in a home health or home care agency.

Nice To Haves

• At least one year of experience working with an EMR and scheduling system.

Responsibilities

• Coordinate Home Care Assistant and Personal Care Assistant assignments as directed by Home Health RNs and the Home Care Supervisor, including schedule changes and communications with participants, families, and home services staff.
• Complete required authorizations in the electronic medical records system for approved personal care and home health services.
• Ensure medical records for clinicians, home care aides, vendors, and in-home staff visits are received and uploaded into the electronic health record.
• Implement and improve procedures such as making reminder calls, clarifying instructions, and sending necessary materials to support successful experiences for providers, staff, and participants.
• Maintain current and accurate records of scheduled home health appointments and home and personal care assignments, including scheduled times and outcomes.
