About The Position

Above and Beyond Monticello, a Trinity Health at Home company, is seeking a compassionate and dedicated Home Care Primary Care / Registered Nurse (RN) to provide high-quality, patient-centered care in patients’ homes. The role supports patients in maintaining their health, independence, and dignity. The coverage area is approximately between Cedar Rapids and Monticello.

The schedule is Monday through Friday, 8 a.m. to 5 p.m., with a rotating weekend every fourth or fifth weekend. There are no on-call requirements. The employer describes the work as consistent, with steady assignments and guaranteed hours, and uses the Epic EMR system for documentation and communication.

Requirements

• Graduate of an approved nursing program.
• Current RN license in the state of practice.
• At least one year of professional nursing experience.
• Strong computer and technology skills.
• Valid driver’s license and reliable transportation.
• Commitment to the organization’s mission, ethics, and compliance standards.

Nice To Haves

• BSN preferred.
• Home health experience is a plus.

Responsibilities

• Conduct comprehensive assessments of patients’ health status and needs.
• Develop and implement individualized care plans in collaboration with interdisciplinary teams.
• Administer medications and treatments and monitor patient responses.
• Educate patients and families on disease management and health maintenance.
• Monitor vital signs and manage chronic conditions.
• Accurately document patient care activities.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Provide emotional support and counseling to patients and families.
• Ensure compliance with healthcare regulations and standards.
• Participate in quality improvement initiatives and ongoing education.
