About The Position
The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) works under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The role supports patients and caregivers through personalized care and collaboration with physicians and registered nurses. The position receives compensation through per-visit-point pay for visits and hourly pay for certain non-visit activities, such as orientation. Annual total cash compensation estimates assume full-time employment at 40 hours per week and full productivity. A $10,000 sign-on bonus is available to external candidates.
Requirements
- Current LPN license specific to the state(s) where assigned to work.
- Current CPR certification.
- At least 1 year of LPN or LVN experience.
- Valid driver's license, reliable transportation, and liability insurance.
Responsibilities
- Verify the plan of care with the RN before each visit.
- Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient's condition changes.
- Teach patients and caregivers self-care techniques.
- Check vital signs.
- Review medications and perform medication reconciliation at each visit.
- Perform physician-ordered specialized procedures, such as Foley catheter changes.
- Prepare equipment and materials for treatments using aseptic technique.
- Complete documentation accurately and on time.
