About The Position

Amedisys is hiring a full-time Home Health Registered Nurse case manager for the Fountain Inn/Clinton area, with training and orientation in Clinton, South Carolina. The role offers a flexible four-day workweek and a $10,000 sign-on bonus. The nurse helps patients heal at home by assessing their needs, coordinating and providing care, supporting independence, and working with the care team. Compensation is $70,200 to $78,000 annually and may vary based on geographic market, job-related knowledge, skills, and experience.

Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP Clinical.
• Current RN license for the state or states assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of RN clinical experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Refer patients to other disciplines when indicated by their needs, or document the rationale for not doing so.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including instruction in self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
