About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Physical Therapist – HAC helps patients restore function, improve mobility, and enhance quality of life. The role delivers skilled physical therapy under a physician-directed plan of care, including patient assessment, treatment planning, intervention, and care coordination within an interdisciplinary care model. Requirements
- Current, unencumbered license to practice physical therapy in the assigned state.
- Graduate of a CAPTE-accredited physical therapy program.
- At least 1 year of physical therapy experience, or less than 1 year of experience with participation in an approved residency or mentorship program.
- Current CPR certification.
- At least 1 year of experience in clinical assessment, documentation, and care coordination.
- Must pass a drug test before beginning employment.
Nice To Haves
- At least 1 year of experience in home health, hospital-at-home, or community-based therapy settings.
- At least 1 year of experience working within interdisciplinary healthcare teams.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized physical therapy plans of care in collaboration with physicians.
- Provide skilled physical therapy interventions and modify treatment plans based on patient progress and clinical needs.
- Perform ongoing reassessments, including weekly evaluations, to support care planning and physician coordination.
- Collaborate with interdisciplinary teams to coordinate care and communicate patient progress and changes in condition.
- Maintain accurate, timely documentation in compliance with clinical, regulatory, and documentation standards.
