About The Position

Health Care 4 All, Inc. is seeking a Part-Time Clinical Manager to oversee and support the delivery of safe, high-quality home health services. The Clinical Manager will provide clinical supervision, coordinate patient care, monitor staff performance, and ensure compliance with applicable home health, Medicare, and Medicaid requirements. At Health Care 4 All, we are dedicated to helping our clients live independently and safely in their home, offering a wide range of home care services tailored to each client’s individual needs. We ensure that our clients and their families are informed and involved in planning their care. We work closely with our clients and their families to ensure they receive proper care and feel comfortable and safe with our team of caregivers. Requirements

  • Current, unrestricted RN license in the state of employment.
  • Minimum 1 years of home health/home care experience.
  • Minimum 1 year of clinical supervisory or leadership experience.
  • Strong clinical assessment, leadership, and organizational skills.
  • Knowledge of Medicare and Medicaid home health requirements.
  • Valid driver's license and reliable transportation.
  • Current CPR/First Aid certification.

Nice To Haves

  • Experience with geriatric and/or pediatric patients preferred.
  • Bilingual in English and Spanish preferred.

Responsibilities

  • Provide clinical oversight and supervision of nursing and field staff.
  • Coordinate and monitor patient care services and individualized plans of care.
  • Review patient assessments, clinical documentation, and care plans.
  • Conduct patient assessments and home visits as needed.
  • Ensure care is delivered according to physician orders, standards of practice, and regulatory requirements.
  • Monitor patient progress and address changes in condition or care needs.
  • Support staff with clinical guidance and problem-solving.
  • Ensure compliance with Medicare, Medicaid, state regulations, and agency policies.
  • Participate in quality assurance and clinical performance improvement activities.
  • Participate in on-call coverage as assigned.
