About The Position

VitalCaring is seeking a Clinical Care Manager (RN) to lead coordination of care and clinical decision-making for home health patients. The role guides patients through the full care journey, working across clinicians, caregivers, physicians, families, and internal teams to support high-quality, compliant care and strong patient outcomes. It is a full-time, salaried position with flexibility and independent day-to-day management, supported by leadership and established processes. In-home visits may be needed.

Requirements

• Active RN license in the state of practice.
• Graduate of an accredited nursing program.
• Strong clinical judgment and ability to manage multiple priorities.
• Confident communication and ability to collaborate across disciplines.
• Comfort working independently in a fast-paced, field-based environment.
• Ability to manage complex patient cases and coordinate across teams.
• Confidence making clinical decisions and driving outcomes.
• Strong organizational skills and attention to detail in documentation and compliance.
• Ability to proactively identify issues and take action.
• Accountability, leadership, and compassion.

Nice To Haves

• BSN preferred.
• Home Health experience strongly preferred.
• Experience in care coordination, case management, or clinical leadership preferred.

Responsibilities

• Coordinate and integrate patient care across clinicians, caregivers, and providers.
• Manage physician orders, documentation, and workflows within the EMR.
• Partner with Intake to support timely admissions and reduce referral delays.
• Conduct ongoing patient assessments and ensure alignment with the plan of care.
• Review clinical reports, including occurrences, infection control, and on-call reports, and take appropriate action.
• Lead case conferences, recertifications, and discharge planning.
• Ensure compliance with clinical, state, and federal regulations.
• Serve as a liaison between physicians, patients, families, and internal teams.
• Support care delivery through in-home visits as needed.
