About The Position

VitalCaring is hiring a Clinical Care Manager (RN) to lead and coordinate home health care across the patient journey. The role guides clinical decisions, coordinates care among patients, providers, caregivers, and teams, and supports quality, compliant care aligned with patient needs. It is a full-time salaried position with flexibility and paid time off. The manager works independently in a fast-paced, field-based environment, with in-home visits as needed.

Requirements

• Active RN license in the state of practice.
• Graduate of an accredited nursing program.
• Strong clinical judgment and ability to manage multiple priorities.
• Confident communication and ability to collaborate across disciplines.
• Ability to work independently in a fast-paced, field-based environment.

Nice To Haves

• BSN preferred.
• Home health experience strongly preferred.
• Experience in care coordination, case management, or clinical leadership preferred.
• Proven ability to manage complex patient cases and coordinate across teams.
• Confidence making clinical decisions and driving outcomes.
• Strong organizational skills and attention to detail in documentation and compliance.
• Ability to proactively identify issues and take action.
• Accountability, leadership, and compassion.

Responsibilities

• Coordinate and integrate patient care across clinicians, caregivers, and providers.
• Manage physician orders, documentation, and workflows within the EMR.
• Partner with Intake to support timely admissions and reduce referral delays.
• Conduct ongoing patient assessments and ensure alignment with the plan of care.
• Review clinical reports, including occurrences, infection control, and on-call reports, and take appropriate action.
• Lead case conferences, recertifications, and discharge planning.
• Ensure compliance with clinical, state, and federal regulations.
• Serve as a key liaison among physicians, patients, families, and internal teams.
• Support care delivery through in-home visits as needed.
