About The Position

VitalCaring is seeking a Registered Nurse to serve as a Clinical Care Manager in home health. The role leads coordination of care, guides clinical decisions, and ensures patients receive appropriate, high-quality care throughout their journey. The manager works across patients, providers, caregivers, and internal teams in a fast-paced, field-based environment, with autonomy supported by leadership and established processes. This is a full-time salaried role with flexibility and paid time off.

Requirements

• Active RN license in the state of practice.
• Graduate of an accredited nursing program.
• Strong clinical judgment and ability to manage multiple priorities.
• Confident communication and ability to collaborate across disciplines.
• Comfort working independently in a fast-paced, field-based environment.

Nice To Haves

• BSN preferred.
• Home health experience strongly preferred.
• Experience in care coordination, case management, or clinical leadership preferred.
• Proven ability to manage complex patient cases and coordinate across teams.
• Confidence making clinical decisions and driving outcomes.
• Strong organizational skills and attention to detail in documentation and compliance.
• Ability to proactively identify issues and take action.
• A balance of accountability, leadership, and compassion.

Responsibilities

• Coordinate and integrate patient care across clinicians, caregivers, and providers.
• Manage physician orders, documentation, and workflows within the EMR.
• Partner with Intake to support timely admissions and reduce referral delays.
• Conduct ongoing patient assessments and ensure alignment with the plan of care.
• Review clinical reports, including occurrences, infection control, and on-call reports, and take appropriate action.
• Lead case conferences, recertifications, and discharge planning.
• Ensure compliance with clinical, state, and federal regulations.
• Serve as a liaison among physicians, patients, families, and internal teams.
• Support care delivery through in-home visits as needed.
