About The Position

The Health Services Clinic Manager (RN) is a BSN-prepared registered nurse responsible for coordinating comprehensive patient care, providing health education and self-management support, and overseeing day-to-day clinical and operational functions. The Health Services Clinic Manager (RN) serves as a key link among patients, providers, specialists, community resources, and clinic staff. The position combines direct patient care, care coordination, health education, population health, quality improvement, staff leadership, workflow management, and clinic operations, with a focus on providing timely, safe, culturally responsive, equitable, and patient-centered care, particularly for underserved, uninsured, and underinsured populations.

Requirements

  • BSN-prepared registered nurse
  • Minimum of 2-3 years of professional nursing experience, preferably in primary care, family practice, community health, public health, care coordination, or a related setting.
  • Experience with chronic disease management, patient education, and care coordination.
  • Strong clinical assessment, nursing judgment, critical-thinking, and problem-solving skills.
  • Knowledge of primary care, chronic disease management, care coordination, and preventive health.
  • Excellent organizational, communication, and interpersonal skills.
  • Ability to work effectively with individuals from diverse cultural, socioeconomic, and linguistic backgrounds.
  • Understanding of health literacy and culturally responsive care.
  • Ability to prioritize competing clinical and operational responsibilities and work independently and collaboratively.
  • Proficiency with EHR systems and standard office technology.
  • Commitment to confidentiality, professional boundaries, and the mission of nonprofit/community healthcare.

Responsibilities

  • Coordinate comprehensive care for patients with acute, chronic, complex medical, and social needs, including assessment, triage, care planning, transitions of care, and follow-up.
  • Review clinical information, including medical records, laboratory and diagnostic results, referrals, and hospital/ED discharge information, to identify and address follow-up needs.
  • Coordinate referrals, diagnostic testing, medications, and communication among patients, providers, pharmacies, specialists, hospitals, and community resources.
  • Identify high-risk patients and barriers to care and implement appropriate interventions to improve access, adherence, continuity, and outcomes.
  • Support medication reconciliation and adherence and follow up on abnormal results in accordance with provider recommendations and clinic protocols.
  • Maintain timely, accurate, and complete documentation in the EHR and appropriately escalate urgent or emergent clinical concerns.
  • Provide individualized, culturally and linguistically appropriate education related to diagnoses, medications, treatment plans, prevention, nutrition, physical activity, and healthy lifestyle behaviors.
  • Support chronic disease prevention and management and promote preventive care, including immunizations, screenings, and health maintenance.
  • Help patients establish achievable self-management goals and use evidence-based approaches, including motivational interviewing, to support behavior change and treatment adherence.
  • Develop or adapt patient education materials and participate in group education, community health programs, and outreach activities as appropriate.
  • Oversee and coordinate daily clinical operations and patient flow in collaboration with the Medical Director and COO to promote safe, efficient, and patient-centered care.
  • Supervise, train, mentor, and support clinical support staff as assigned, including delegation, competency development, performance feedback, and accountability.
  • Develop, implement, and maintain clinical workflows, protocols, standing orders, safety procedures, and operational processes.
  • Coordinate staffing, scheduling, clinic readiness, supplies, equipment, medications, vaccines, and emergency resources.
  • Promote effective communication and teamwork across clinical, administrative, and volunteer staff and identify and resolve operational or workflow issues.
  • Monitor clinical, preventive, operational, and population health measures and identify gaps in care or opportunities for improvement.
  • Conduct or support patient outreach, chart audits, quality assurance activities, and interventions to improve completion of screenings, laboratory monitoring, referrals, immunizations, and chronic disease care.
  • Participate in quality improvement, patient safety, and evidence-based practice initiatives and track relevant outcomes and metrics.
  • Assist with regulatory, accreditation, grant, and programmatic reviews and support implementation of organizational quality and safety priorities.
  • Practice within the RN scope of practice and comply with applicable federal, state, and local laws, regulations, and professional standards.
  • Maintain compliance with HIPAA, OSHA, infection prevention and control requirements, and organizational policies.
  • Promote patient confidentiality, safety, accurate documentation, appropriate clinical escalation, and timely reporting of incidents or safety concerns.
  • Collaborate with leadership to develop, implement, and update clinical policies and procedures and support organizational risk-management and compliance activities.
