Health Risk Assessments - Nurse Practitioner (NP) and Physician Assistants (PA)
About The Position
This flexible PRN opportunity is for Nurse Practitioners and Physician Assistants conducting in-home Health Risk Assessments (HRAs) for Medicare Advantage members. The role offers autonomous work and the opportunity to supplement income while contributing to patient care, without ongoing treatment responsibilities. Clinicians create their own schedule, with a minimum availability of eight full 7.5-hour days per month. The position involves local travel to patients within the clinician’s county and nearby areas, generally within roughly 60 minutes of home. Documentation is completed through a proprietary EMR system using a company-provided iPad. Malpractice insurance and collaborating physicians are included. Onboarding may take 3 to 4 weeks.
Requirements
- Active Nurse Practitioner or Physician Assistant license.
- Pediatrics, PMHNP, and Women’s Health specialties are excluded.
- Availability for at least eight full 7.5-hour days per month.
- Willingness to travel within roughly 60 minutes of home.

Responsibilities
- Complete comprehensive in-home Health Risk Assessments for Medicare Advantage patients.
- Review each patient’s medical history, current medications, lifestyle, and key social factors.
- Conduct basic physical and mental health evaluations.

- Travel locally to patients in the clinician’s county and nearby areas.
