Health Risk Assessments - Nurse Practitioner (NP) and Physician Assistants (PA)
About The Position
Nurse Practitioners and Physician Assistants conduct in-home Health Risk Assessments (HRAs) for Medicare Advantage members. This flexible PRN opportunity allows clinicians to work autonomously and supplement their income without ongoing treatment responsibilities. Assessments include reviewing patients’ medical histories, current medications, lifestyles, and social factors; conducting basic physical and mental health evaluations; and sharing preventive care education. The role does not involve prescribing, on-call shifts, or follow-up care. Clinicians travel locally to patients in their county and nearby areas. Documentation is completed using a company-provided iPad and the company’s proprietary EMR system. Malpractice insurance and collaborating physicians are included. The position is offered as a 1099 contractor role, with quick onboarding that may take 3 to 4 weeks.
Requirements
- Active Nurse Practitioner or Physician Assistant license.
- Pediatrics, PMHNP, and Women’s Health specialties are excluded.
- Availability for a minimum of eight full 7.5-hour days per month.
- Willingness to travel within roughly 60 minutes of home.
Responsibilities
- Complete comprehensive in-home Health Risk Assessments for Medicare Advantage patients.
- Review each patient’s medical history, current medications, lifestyle, and key social factors.
- Conduct basic physical and mental health evaluations.
- Share preventive care education; ongoing management is not required.
- Travel locally to patients in the clinician’s county and nearby areas.
- Complete documentation using the company-provided iPad and proprietary EMR system.
