About The Position

Nurse Practitioners and Physician Assistants conduct in-home Health Risk Assessments (HRAs) for Medicare Advantage members. This flexible PRN opportunity allows clinicians to work autonomously and supplement their income without responsibility for ongoing treatment. Clinicians create their own schedule, with a minimum availability of eight full 7.5-hour days per month. The role includes competitive pay with performance incentives, and provides malpractice insurance, collaborating physicians, a company-provided iPad, and access to a proprietary EMR system. There are no prescribing duties, on-call shifts, or follow-ups. Onboarding may take 3 to 4 weeks. The position is offered as a 1099 contractor role. Clinicians travel locally to patients in their county and nearby areas, generally within roughly 60 minutes of home. The posting notes that contractors may be able to deduct $0.725 per mile in 2026 and advises consulting a tax advisor.

Requirements

Active Nurse Practitioner or Physician Assistant license.
At least eight full 7.5-hour days of availability per month.
Willingness to travel within roughly 60 minutes of home.
Pediatrics, Psychiatric-Mental Health Nurse Practitioner (PMHNP), and Women's Health specialties are excluded.

Responsibilities

Complete comprehensive in-home Health Risk Assessments for Medicare Advantage patients.
Review each patient's medical history, current medications, lifestyle, and key social factors.
Conduct basic physical and mental health evaluations.
Share preventive care education; ongoing management is not required.
Travel locally to patients in the clinician's county and nearby areas.
