The Health Information Assistant supports staff and the Health Information Management (HIM) Department by helping monitor and maintain complete health records for all persons served. Accuracy and thoroughness are important because records may affect the facility’s legal liability. The role requires professional interaction with staff at all levels and with stakeholders, including persons served and their families. The position reports to the Health Information Coordinator and works closely with the Director of Nursing, Program Director/Clinical Directors, and other department heads. The Health Information Assistant receives consultation, advice, training, and mentorship on technical record matters from the Health Information Coordinator and, as required by program-specific regulations, Crestwood’s corporate Health Information Department. The position is based at Crestwood Center San Jose Psychiatric Health Facility. The campus provides a spacious environment with living rooms, a welcoming room, visiting space, a serenity room, a dining room, and a courtyard. The facility serves clients requiring stabilization during acute psychiatric crises. The posting also describes Crestwood’s Mental Health Rehabilitation Centers as providing psychosocial rehabilitation in secure residential settings, generally for brief stays, with a focus on hope, empowerment, community and natural-support inclusion, potential, and psychiatric stabilization.
