About The Position
The Family Support Specialist enhances the independence of people with disabilities and facilitates their care. The role provides information and support to individuals and families, assists them in accessing appropriate services, and helps connect participants with community resources to increase inclusion.
Requirements
- Basic word-processing and email skills.
- A valid driver’s license and transportation necessary to meet program needs.
Nice To Haves
- High school diploma or GED plus three years of experience working with individuals with disabilities.
- Associate’s degree in a human services field plus two years of experience working with individuals with disabilities.
- Experience working with individuals with disabilities.
Responsibilities
- Make home and collateral visits to individuals and their families to provide information and support and assist with acquiring appropriate services.
- Provide information, advocacy, and referral services by consulting with individuals, families, professionals, and programs, and conducting related research.
- Assist families and individuals through the OPWDD eligibility process.
- Monitor budgets independently and accurately.
- Research community resources and provide linkages to increase participants’ inclusion in their communities.
