About The Position

The Community Liaison serves as the primary relationship manager between Enhabit and its referral sources, including physicians and their practices, hospitals, health systems, inpatient facilities, senior living communities, professional associations, and other healthcare organizations and professionals. The goal is to help people navigate post-acute care transitions, improve patient outcomes, support care transitions, and promote exceptional quality and patient and referral-source satisfaction. The position requires a bachelor’s degree in healthcare administration, marketing, or a related field, or graduation from an approved school of nursing, therapy, or social work. Equivalent years of work experience may be considered in lieu of degree requirements. Candidates need at least two years of experience in healthcare business development, community relations, marketing, or sales, and demonstrated understanding of laws and regulatory guidelines governing Medicare-certified home health and hospice operations. The role also requires a valid driver’s license, automobile liability insurance, dependable transportation, and strong communication, interpersonal, and presentation skills. In-office attendance is essential except for designated full-time remote positions; occasional remote work to accommodate temporary personal needs or crises does not change this requirement.
