The Community Liaison serves as the primary relationship manager between Enhabit and referral sources, including physicians and their practices, hospitals, health systems, inpatient facilities, senior living communities, professional associations, and other healthcare organizations and professionals. The role supports referral growth by educating referral sources, patients, families, caregivers, and community partners about company services and program requirements. This helps people navigate post-acute care transitions, with the goals of improving patient outcomes, supporting care transitions, and promoting exceptional quality and patient and referral-source satisfaction. The position requires a bachelor's degree in healthcare administration, marketing, or a related field, or graduation from an approved school of nursing, therapy, or social work; equivalent work experience may substitute for degree requirements. The role also requires at least two years of relevant business development, community relations, marketing, or sales experience, as well as knowledge of laws and regulatory guidelines governing Medicare-certified home health and hospice operations. Clinical license requirements apply to clinical candidates. In-office attendance is essential except for designated full-time remote positions; occasional remote work may be permitted for temporary personal needs or crises beyond the company's control.
