About The Position

Signify Health, part of CVS Health®, is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships. The clinician will conduct comprehensive health evaluations, including medication and medical history review, physical evaluations, and potentially diagnostic tests. The role utilizes an iPad with an intuitive clinical workflow and does not require prescribing medication, ordering lab tests, or altering current treatment regimens. The primary workplace is the private residence of health plan members, which may present diverse conditions including household pets, secondhand smoke, and other airborne allergens or irritants. Candidates must be able to perform duties comfortably and safely in these environments.

Requirements

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Able to cover the following primary locations: Seattle/ King County

Responsibilities
