About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians meet with members in their homes, build trusted relationships, and offer flexible, comprehensive, personalized evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and—when ordered by the member’s health plan—additional diagnostic tests such as diabetic eye exams or spirometry. Clinicians use an iPad and a clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen.

The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The role may also involve local travel and travel within licensed states, as needed, and virtual visits as needed. The position is based in the Greater Boston Area, covering Bristol, Norfolk, Plymouth, and Suffolk.

The position is part-time, with anticipated hours of 29.5 per week. The typical pay range is $46.03–$99.14. Part-time employees qualify for select benefits, malpractice insurance coverage during Signify Health-related activities, supplies, and other perks. Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.
• Ability to cover the Greater Boston Area, including Bristol, Norfolk, Plymouth, and Suffolk.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic and preventive services when indicated, including services such as spirometry, diabetic eye exams, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Build trusted relationships with members and help connect them to appropriate care.
• Be flexible to travel locally and within licensed states, as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
