About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations in Saratoga, New York. The clinician engages with health plan members in their homes, helps bridge gaps in care, and builds trusted relationships to support better health outcomes. Visits take about 45 minutes and include time to answer health questions, review medications and medical history, and conduct a physical evaluation. Additional diagnostic tests may be performed when ordered by the member’s health plan, such as diabetic eye exams or spirometry. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a member’s current treatment regimen.

The clinician may travel locally and within licensed states, as needed, and conduct virtual visits when required. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. Candidates must be able to perform duties safely and comfortably in these environments.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in members’ homes, which may contain pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities
