In-Home Physician - Part Time - Lancaster PA
About The Position
Signify Health, part of CVS Health®, is seeking a Part Time Physician to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The evaluations are comprehensive, lasting about 45 minutes, and include a medication and medical history review, a physical evaluation, and potentially diagnostic tests. Physicians will use an iPad with an intuitive clinical workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can vary in conditions, and candidates must be comfortable working in diverse home environments, potentially including pets, secondhand smoke, and airborne allergens.

Responsibilities
- Provide In-Home Health Evaluations.
- Engage with people in the comfort of their own homes.

- Conduct flexible, comprehensive, and personalized health evaluations.
- Connect one-on-one with patients and answer health questions.
- Perform medication and medical history reviews.
- Conduct physical evaluations.
- Perform additional diagnostic tests if ordered by the person’s health plan (e.g., diabetic eye exams, spirometry).
- Use an iPad and an intuitive clinical workflow for each evaluation.

- Participate in efforts to visit millions of people nationwide with the goal of providing connections to the right care for people’s unique needs.
- Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
- Be flexible to travel locally and within licensed states, as needed.
- Conduct virtual visits as needed.
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
