About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in the Greater Boston Area. The role focuses on building trusted relationships, improving health outcomes, and connecting members with care suited to their needs.

Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a person’s current treatment regimen. Virtual visits may also be conducted as needed.

The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other potential airborne allergens or irritants. The role requires flexibility to travel locally and within licensed states as needed. The anticipated schedule is 29.5 hours per week. The typical pay range is $46.03–$99.14 per hour; the actual offer depends on factors including experience, education, and geography.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to cover the Greater Boston Area locations of Bristol, Norfolk, Plymouth, and Suffolk.
• Ability to comfortably and safely perform duties in residences that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by a member’s health plan, including services such as diabetic eye exams, spirometry, and bone density screening.
• Use an iPad and the clinical workflow for evaluations.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
• Engage with members, answer health questions, and help connect them with care suited to their needs.
