In-Home Health - NP or PA (Part Time) - Bridgeport, CT (South Central)
About The Position
Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations to health plan members. The role focuses on building trusted relationships, helping members address their health needs, and bridging gaps in care. Visits typically last about 45 minutes and include one-on-one discussion, answering health questions, reviewing medications and medical history, and conducting a physical evaluation. Additional diagnostic tests may be performed when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or alter a member’s current treatment regimen. The primary workplace is members’ private residences, which may have household pets, secondhand smoke, or other airborne allergens or irritants. Clinicians must be able to perform their duties comfortably and safely in these environments. The role may also involve local travel and travel within licensed states, as needed, and virtual visits as needed.
Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.

Responsibilities
- Conduct in-home health evaluations, typically lasting about 45 minutes.
- Engage with members one-on-one, answer health questions, and help connect them with care suited to their needs.
- Review members’ medications and medical history and perform a physical evaluation.
- Provide additional diagnostic tests when ordered by the member’s health plan, including services such as diabetic eye exams, spirometry, and bone density screening.
- Use an iPad and the clinical workflow for each evaluation.
- Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Fulfill the requirements of the specified clinician role as a Signify Health colleague.
