In-Home Health - NP or PA (Part Time) - Denver, CO - Arapahoe
About The Position
Signify Health, part of CVS Health®, is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The clinician will conduct comprehensive and personalized health evaluations, including medication and medical history review, physical evaluations, and diagnostic tests as ordered. The role utilizes an iPad with an intuitive clinical workflow and does not require prescribing medicine, ordering lab tests, or altering current treatment regimens. The primary workplace is the private residence of health plan members, which may present diverse conditions including household pets, secondhand smoke, and airborne allergens.
Requirements
- Active, unrestricted license(s) in coverage area(s).
- Board certification (required).
- Able to cover the following primary locations: Denver/Arapahoe County.

Responsibilities
- Conduct In-Home Health Evaluations.

- Conduct medication and medical history reviews.
- Perform physical evaluations.
- Perform additional diagnostic tests if ordered by the person’s health plan (i.e., diabetic eye exams, spirometry, etc.).
- Use an iPad and an intuitive clinical workflow for each evaluation.

- Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
- Be flexible to travel locally and within licensed states, as needed.
- Conduct virtual visits as needed.
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.).
