About The Position

Luxor Healthcare is seeking an energetic, relationship-driven Clinical Liaison to support its network of skilled nursing and rehabilitation communities throughout the Kansas City area. The role involves building relationships, coordinating patient transitions, and connecting individuals with post-acute care services. The Clinical Liaison works with hospitals, physicians, case managers, and community partners to support admissions and occupancy growth, and travels between facilities and throughout the Kansas City market.

Requirements

• At least 2 years of admissions, business development, clinical liaison, or related healthcare experience.
• Strong communication, organizational, and relationship-building skills.
• Professional customer service and phone skills.
• Reliable transportation and willingness to travel throughout the Kansas City area.

Nice To Haves

• Experience in skilled nursing, rehabilitation, post-acute care, or hospital settings.
• Knowledge of Managed Care processes.
• LPN license.

Responsibilities

• Develop and maintain referral relationships with hospitals, physicians, case managers, and community organizations.
• Review referrals and coordinate admissions processes, including assessments, documentation, and authorizations.
• Conduct facility presentations and tours for referral sources and families.
• Communicate resident needs to facility teams to support smooth transitions.
• Promote a positive image of the facilities within the community.
• Maintain knowledge of Medicare, Medicaid, and Managed Care processes.
• Travel between facilities and throughout the Kansas City market to support referral development and admissions growth.
