About The Position

Heart’n Soul Hospice seeks a Clinical Liaison Registered Nurse to support its mission of helping patients and families access high-quality, compassionate care during life-limiting illness. The role develops the hospice’s presence in the healthcare community, assesses patients for hospice admission, educates and consults with medical professionals, supports public relations and business development, and helps ensure regulatory compliance. Requirements

Graduate of an accredited diploma, associate, or baccalaureate school of nursing.
Current state license as a Registered Nurse.
Valid driver’s license and car insurance.
Two years of experience as an RN; hospice or healthcare/clinical-setting experience is preferred.

Responsibilities

Develop a hospice presence among hospitals and healthcare providers.
Perform comprehensive nursing assessments and evaluate appropriateness for hospice admission.
Develop, educate, and consult with medical professionals about hospice programs, pain management, and Medicare, Medicaid, and commercial reimbursements.
Establish public relations programs to explain the agency’s services and foster good working relationships with physicians and community agencies.
Implement specific projects and initiatives as directed.
Support new business development and assist Community Education Representatives with new account development when needed.
Perform other duties as required.
Ensure overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
