About The Position

The Clinical Care Coordinator II provides patient care according to age-specific population needs and assists in managing patient care in a physician office setting under the direct supervision of a provider. This position is part of the FPG Nurse Triage Team and is based at FPG’s Centralized Access Center on University Parkway in Lakewood Ranch.

The role is conducted in a call-center environment, with phone calls throughout the entire shift, and does not provide direct patient care. The LPN takes calls from established FPG patients, follows protocols for reported symptoms, and offers guidance in strict adherence to established protocols. The team aims to provide excellent patient care and real-time clinical guidance, working closely with the Clinical Excellence and Operations team, providers, and provider-office care teams to support seamless patient experiences and effective communication.

Requirements

• Graduate of an accredited school of nursing or a diploma program.
• Basic Life Support/CPR certification, or obtain it within 30 days of hire or transfer.
• Florida Licensed Practical Nurse (FL LPN).
• Comfort working in a call-center environment and making phone calls for the entire shift.

Nice To Haves

• One year of clinical experience in an outpatient physician office practice.
• Experience with electronic practice management and electronic health records.
• Demonstrated knowledge of electronic practice management and electronic health records software.
• Skill with Microsoft Office programs, including Word, Excel, Outlook, and PowerPoint.
