Chronic Care Coordinator

Juno Medical is seeking a Chronic Care Coordinator to join their team and implement a new Chronic Condition Monitoring program. The coordinator will be responsible for enrolling and onboarding patients into the program, conducting virtual sessions, reviewing and responding to patient data, and supporting patients in navigating the healthcare system. This position offers a hybrid work arrangement and requires building long-term relationships with patients to help them achieve their health goals.

• Enrolling and onboarding patients into the chronic care management program
• Conducting virtual sessions with patients and providing standardized patient education
• Reviewing and responding to patient data based on care plan parameters
• Supporting patients and their families in navigating the healthcare system
• Managing patient messages and alerts, escalating urgent care needs when necessary
• Communicating and collaborating with the patient's physician and care team
• Achieving program goals including patient engagement and clinical quality measures
• Contributing to the development of chronic care curriculum
• Assisting in the development of the remote monitoring program and onboarding patients into the program
• Performing additional duties and tasks as assigned
