Chronic Care Coordinator

The Chronic Care Coordinator at Juno Medical is responsible for implementing and managing the new Chronic Condition Monitoring program. This program aims to promote positive lifestyle changes and improved health outcomes for patients by providing support and guidance in navigating the healthcare system and tracking key health information. The coordinator will work closely with patients, using various technologies to regularly check-in and coordinate their care across different providers and community partners. Success in this role is driven by cultivating long-term relationships with patients and helping them achieve their health goals through coaching and navigation. The position offers a hybrid work arrangement and supports multiple markets.

• Enrolling and onboarding patients into the chronic care management program
• Conducting virtual sessions with patients and providing standardized patient education
• Reviewing and responding to patient data based on care plan parameters
• Supporting patients and their families in navigating the healthcare system
• Managing patient messages and alerts, escalating urgent care needs when necessary
• Communicating and collaborating with the patient's physician and care team
• Achieving program goals including patient engagement and clinical quality measures
• Contributing to the development of chronic care curriculum
• Assisting with the development of the remote monitoring program and onboarding patients into the program
• Performing additional duties and tasks as assigned

• Reviewing and responding to patient data based on care plan parameters
• Supporting patients in navigating the healthcare system and coordinating necessary care
• Managing patient messages and alerts, escalating urgent care needs when necessary

• Achieving program goals including patient engagement and clinical quality measures
• Contributing to the development of chronic care curriculum
• Assisting with the development of the remote monitoring program and onboarding patients into the program
