About The Position

The CHOICES Care Coordinator helps members achieve personal health and wellness goals and supports their safety at home or in a community setting. The role includes traveling to members’ homes to conduct visits and coordinating with internal and external stakeholders in a fast-paced environment. The position covers members in Maury, Lawrence, Giles, and Lewis counties; applicants are required to live in Maury County, with preference given to applicants who live within 50 miles of the coverage area. The schedule is 8:00 a.m. to 5:00 p.m. EST, with no on-call duties. With management approval, employees may be eligible to work a compressed work week after one year. Employees must remain in the role for 24 months before becoming eligible to apply for other internal positions.

Requirements

• At least 2 years of clinical experience is required; the introductory role description states 3 years of clinical-setting experience.
• Be a registered nurse with an active Tennessee license, or an active license in the state of residence if that state participates in the Nurse Licensure Compact Act; alternatively, hold a Master’s in Social Work and an active, unrestricted LCSW, LMSW, or LAPSW license.
• Live in Maury County, Tennessee.
• Have a valid driver’s license.
• Have basic PC skills, including Microsoft Office and email.
• Demonstrate effective time-management skills.
• Have excellent oral and written communication skills.
• Have strong interpersonal and organizational skills.
• Have exceptional customer service skills.
• Complete a digital literacy assessment.
• Complete a TB skin test if working in the field.
• Meet applicable immunization and associated medical-test requirements.
• If routinely operating a company-owned, personal, or rental vehicle for company business, comply with the BCBST Driver Safety Program, including maintaining a valid driver’s license, meeting applicable auto-insurance requirements when using a personal or rental vehicle, and maintaining an acceptable motor vehicle record.

Responsibilities

• Partner with members and families to identify supports and direct services that help members meet personal goals related to health, employment, and independent or community living.
• Collaborate with clinical and social support colleagues to address each member’s physical, behavioral health, and long-term service needs.
• Conduct thorough, objective, face-to-face visits and assess members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living status and needs.
• Use criteria to authorize appropriate home- and community-based services, confirm the services are provided, and ensure members’ needs are met.
