The CHOICES Care Coordinator helps members achieve personal health and wellness goals and supports their safety at home or in a community setting. The role includes traveling to members’ homes for visits and managing requests and demands from internal and external stakeholders. The ideal candidate is self-motivated, flexible, effective at critical thinking, problem-solving, time management, and prioritizing tasks, and passionate about improving the quality of life for diverse members in their communities. The position serves Sumner and Robertson Counties in Tennessee and follows an 8:00 a.m.–5:00 p.m. EST schedule with no on-call duties. With management approval, employees may be eligible to work a compressed work week after one year. The coordinator partners with members and families to identify supports and direct services aligned with personal goals for health, employment, and independent or community living. They collaborate with clinical and social support colleagues, assess members’ needs through face-to-face visits, and apply authorization criteria for home- and community-based services to verify that services are delivered and needs are met. The position requires a valid driver’s license and, for field-based coordinators, a TB skin test. Various immunizations or associated medical tests may also be required. A digital literacy assessment is required, and employees must spend 24 months in the role before becoming eligible to apply for other internal positions.
