About The Position

One Love Agency is hiring a full-time Behavioral Health Case Manager for our Idaho Falls office, starting immediately. This role involves helping individuals and families through challenging times, such as securing appointments, signing leases, returning to school, or starting new jobs. The agency has been serving clients for over fifteen years across Idaho and Utah, focusing on adults in crisis, children and families needing support, and individuals recovering from addiction. The agency operates on the principle of 'hānai,' meaning to take in and raise as one's own, fostering a family-like environment for both clients and staff. The team emphasizes a collaborative approach where every role is crucial to client success, adhering to three core values: Clients (prioritizing their needs), Compliance (ensuring quality service through documentation), and Community Partners (maintaining trust with external agencies).

Requirements

  • Bachelor's degree in a health and human services field (social work, psychology, counseling, sociology, human services, or related field).
  • Meeting Idaho Behavioral Health Plan case management qualifications.
  • Ability to practice under supervisory protocol.
  • Ability to pass an Idaho Department of Health and Welfare criminal history background check and be cleared before direct client contact.
  • Clear of any state or federal exclusion list (OIG LEIE / SAM).
  • Valid Idaho driver's license, reliable transportation, and current auto insurance.
  • Comfort with electronic health records.
  • Commitment to same-week documentation.

Nice To Haves

  • Licensed candidates (BSW, MSW, LSW, LMSW, LCSW, LPC, LCPC, LMFT, or RN) are strongly encouraged to apply and may qualify for expanded scope.
  • Experience in case management, CBRS, YES, or equivalent community-based behavioral health.
  • Familiarity with the Idaho Falls resource map, including schools, housing, benefits, treatment, recovery, food, transportation, and legal/medical services.
  • Understanding of system processes: eligibility, CDAs, plans of service, units and authorizations, medical necessity, and IBHP expectations.
  • CANS certification (Idaho YES) for the youth caseload.
  • Experience with adults with serious mental illness, co-occurring substance use, or developmental disabilities.
  • Bilingual or multilingual ability.
  • Trauma-informed approach.
  • Culturally humble.
  • Ability to remain steady during challenging situations.

Responsibilities

  • Assessing strengths, needs, and goals alongside the client and their support network.
  • Developing and maintaining a plan of service.
  • Coordinating various services including medical and behavioral health care, school supports, housing, benefits, employment, transportation, and natural supports.
  • Advocating for clients within systems that may be resistant to change.
  • Tracking client progress and adjusting the plan as needed.
  • Completing timely and accurate documentation, tied to the plan and CDA, suitable for Medicaid or IBHP review.
  • Working under supervisory protocol with licensed clinicians and alongside CBRS specialists, therapists, and peer support staff.
  • Reporting to the Idaho Falls Director.
