About The Position

Mercy Rehabilitation Hospital, Northwest Arkansas, operated jointly with Lifepoint Health, is hiring a Case Manager for inpatient rehabilitation. The role focuses on coordinating interdisciplinary care and discharge planning to support patient progress and effective transitions. The Case Manager documents care activities and completes the Individual Plan of Care in accordance with CMS guidelines, communicates progress and discharge plans to caregivers, and collaborates with clinical and health information management colleagues. The position is full-time, day schedule, at the Rogers, Arkansas location.

Requirements

• Active licensure as an RN, Social Worker (LSW), Respiratory Therapist, PT, OT, or SLP.
• At least 2 years of case management or social work experience in an inpatient setting; rehabilitation experience is strongly preferred.
• Strong communication, organization, and collaboration skills.
• Proficiency in Microsoft Office.

Nice To Haves

• Case Management or Rehab Nursing certification, such as CCM, ACM, RN-BC, or ARN.
• Rehabilitation experience.

Responsibilities

• Lead interdisciplinary care coordination and discharge planning.
• Identify barriers to patient progress and help develop solutions with the care team.
• Document care activities and complete the Individual Plan of Care (IPoC) according to CMS guidelines.
• Facilitate weekly team conferences and family meetings.
• Communicate patient progress and discharge plans to caregivers.
• Review Case Mix Groups (CMGs) and collaborate with Health Information Management (HIM) on comorbidities.
• Support additional departmental needs as assigned.
