About The Position

Valley’s EPIC residential program provides individuals with the support, structure, and resources needed to build a foundation for lasting recovery and independence. As a Residential Case Manager, you'll help clients navigate services, achieve personal goals, and overcome barriers through targeted case management, psychosocial rehabilitation, and ongoing support. Your work will play an essential role in empowering clients to make meaningful progress and move toward a healthier future. The Residential Case Manager is a certified position that provides targeted case management and psychosocial rehabilitative services as indicated by the care plan to support clients in residential settings to meet their individualized goals. Certified Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.

Requirements

  • Case Manager certification (post hire as needed - required within 120 days of start date)
  • CPR certification (post hire as needed)
  • Minimum age of 21 and a current driver’s license (for driving positions)

Nice To Haves

  • Previous social services or behavioral health experience
  • Bachelor’s degree in Social Services, Psychology, or related field

Responsibilities

  • Provides targeted case management services as described by the Medicaid manual within residential programs
  • Supports clients in following individualized care plans and identifying case management goals
  • Completes documentation on services provided within Valley’s standards
  • Meets productivity expectations as assigned by supervisor
  • Distributes medications as necessary
  • Observes and collects client urine samples as necessary
  • Transports clients to appointments as necessary
