About The Position
This position is for SOMERSET County. About the Role: Are you a problem-solver with a passion for advocacy? As a Targeted Case Manager, you won't just be managing a caseload—you’ll be the bridge between our participants and the life-changing resources they need. You will work on the front lines, helping individuals with mental illness, navigate healthcare, housing, and social systems to achieve independence and wellness.
Requirements
- Bachelor’s degree in Social Work, Psychology, Sociology, or a related human services field.
- Ability to juggle multiple priorities and deadlines.
- Deep understanding of the challenges faced by vulnerable populations.
- Valid driver’s license and reliable transportation for community-based visits.
Responsibilities
- Conduct comprehensive assessments of participants needs and develop individualized, person-centered service plans.
- Act as a fierce advocate for participant’s rights and access to state, federal, and local benefits.
- Link clients with medical, social, and educational providers; monitor service delivery to ensure it meets the participant’s evolving goals.
- Provide immediate support and resource linkage during critical situations.
- Maintain high-quality, clinical documentation in compliance with Medicaid standards.
