About The Position

The Specialty Case Manager (Bilingual Spanish/English) is part of the Social Services department and supports patients at Urban Health Plan’s Bronx-based health centers, with an emphasis on the behavioral health department. Behavioral health services provide children and adults with comprehensive therapy and outpatient psychiatric support. The Case Manager helps patients access social assistance and services through general screenings, referrals, advocacy, and case management support.

Requirements

Bachelor’s degree in Social Work, Psychology, Human Services, or a related field of study.
Bilingual Spanish/English proficiency is required.
Proficiency with Microsoft Office.
Team-player attitude and willingness to go the extra mile to support patients.
Commitment to excellence and providing quality service to patients.
No previous experience is required beyond the bachelor’s degree.

Responsibilities

Provide case management assistance to patients, including assigned populations.
Prepare letters for patients related to public assistance, housing, the IRS, and schools.
Support completion of prior authorizations.
Provide community-based resources according to site, department, or program needs.
Advocate on patients’ behalf to obtain resources or services, including specialized services.
Work closely with departmental coordinators and team members to support patients.
