The Case Manager uses evidence-based practices, including harm reduction, housing first, and trauma-informed care, to support clients’ housing stability, self-sufficiency, and community integration. The role partners with clients experiencing homelessness to assess and address barriers to housing placement. Once clients find appropriate housing, the Case Manager implements interventions to support housing retention and coordinates services to meet individualized needs. The Case Manager also helps clients develop independent living and other life skills needed to transition from homelessness into permanent housing. The position maintains a caseload of approximately 20 high-acuity clients, or up to 40 low-acuity clients, and works collaboratively with clients, treatment providers, property managers, and multidisciplinary partners. It is based at the NoHo location and is 100% onsite, with a combination of field and office work. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m. The role is full-time, non-exempt, and non-management, and reports to the Program Manager.
