About The Position

Comunilife is hiring experienced Case Managers for its OMH Scatter Program in Queens. The Case Manager provides case management services to a caseload of approximately 22 clients, prepares comprehensive assessments and service plans, and conducts required home visits and fieldwork. This is a non-remote position involving office work and some fieldwork, with a five-day, 35-hour work week and a graveyard shift. Requirements

Bachelor’s degree in human services or a related field and two years of case management work experience; alternatively, an associate degree in human services or a related field and four years of case management work experience; or a high school diploma and six years of case management work experience.
Direct service experience with people living with HIV/AIDS, substance abuse, serious mental illness, and homelessness.
Excellent oral and written communication skills.
Proficiency in Google Workspace programs and Microsoft Office Suite.
Knowledge of HRA Congregate Housing Program guidelines.
Familiarity with AWARDS, Child and Children Integrated Reporting System (CAIRS), and Coordinated Assessment and Placement System (CAPS).
Available for a five-day, 35-hour work week and a graveyard shift.

Nice To Haves

Credentialed Alcohol and Substance Abuse Counseling credential (CASAC-T or CASAC).
Bilingual English/Spanish.

Responsibilities

Complete comprehensive assessments, service plans, and progress notes.
Initiate and continuously assess and monitor clients’ needs for referrals and services.
Provide psychoeducation to clients on topics such as substance abuse and entitlements.
Provide individual and group workshops.
Collaborate with interdisciplinary teams to ensure comprehensive, client-centered services are delivered and documented.
Conduct home visits and fieldwork.
