About The Position

Comunilife is seeking experienced Case Managers for its HRA Scatter Program in the Bronx. The role provides case management services to a caseload of approximately 22 clients, including preparing comprehensive assessments and service plans. Home visits and fieldwork are required. This is a non-remote position involving office work and some fieldwork, with travel of up to 50%. The schedule is five days per week and 35 work hours per week.

Requirements

Bachelor’s degree in human services or a related field and two years of case management work experience; alternatively, an associate degree in human services or a related field and four years of case management work experience; or a high school diploma and six years of case management work experience.
Direct service experience with people living with HIV/AIDS, substance abuse, serious mental illness, and homelessness.
Excellent oral and written communication skills.
Proficiency in Google Workspace and Microsoft Office Suite.
Familiarity with AWARDS, Child and Children Integrated Reporting System (CAIRS), and Coordinated Assessment and Placement System (CAPS).
Ability to work a five-day, 35-hour work week and perform home visits and fieldwork.

Nice To Haves

Credentialed Alcohol and Substance Abuse Counseling certification (CASAC-T or CASAC).
Knowledge of HRA Congregate Housing Program guidelines.
Bilingual English/Spanish.

Responsibilities

Complete comprehensive assessments, service plans, and progress notes.
Initiate and continuously assess and monitor clients’ needs for referrals and services.
Provide psychoeducation to clients on topics such as substance abuse and entitlements.
Provide individual and group workshops.
Collaborate with interdisciplinary teams to ensure comprehensive, client-centered services are delivered and documented.
Conduct home visits and fieldwork.
