About The Position

Fraser is hiring Case Managers to serve individuals receiving CADI/BI and DD waiver services, acting as client advocates and connecting clients with community resources and services. The role provides client-centered case management and calls for compassion, organization, strong documentation and case-noting skills, and a commitment to clients’ success. Caseloads are typically about 35 clients for CADI/BI waiver case managers and about 43 clients for DD waiver and non-waiver case managers. Case managers generally meet with clients two to three times per year; client meetings may occur two to three times per week. CADI/BI caseloads are mostly adults with a small number of children, while DD caseloads include both children and adults. The position supports Ramsey County and requires residence in Minnesota or the Twin Cities metro area. It is Monday through Friday during standard business hours. The role becomes fully remote after the first three to six months of onboarding, and travel to meet clients is required. Starting pay is $50,000 to $55,000 per year, depending on qualifications, with a $1,500 hiring bonus for external candidates.
