About The Position

Avivo’s Ending Homelessness division helps individuals and families work toward stable housing through Housing First, harm reduction, and trauma-informed approaches. The temporary Case Manager in the Scattered Site Housing program supports individuals and families experiencing homelessness, including families referred through Hennepin County shelters, as they secure and maintain housing. The role builds supportive relationships, connects participants with resources, and helps them navigate housing challenges with dignity and compassion. This is a temporary, non-permanent assignment supporting short-term program needs, scheduled for 32 hours per week. The position is based at 1826 Chicago Ave in Minneapolis, Minnesota. Temporary employees are not eligible for Avivo’s employee benefits package. Approved work-related travel is eligible for mileage reimbursement.

Requirements

High school diploma or GED.

Responsibilities

Provide individualized case management and monthly face-to-face support to participants.
Build trusting relationships through compassionate, trauma-informed, and culturally responsive services.
Partner with individuals and families to develop housing goals and personalized stability plans.
Connect participants with community resources, including healthcare, mental health services, and recovery services.
Assist participants with obtaining essential documents, securing housing, and navigating housing barriers.
Educate participants on tenant rights, housing responsibilities, and eviction prevention.
Collaborate with landlords, community partners, and service providers to support housing stability.
Maintain accurate and timely case notes, assessments, and required documentation.
Work collaboratively with team members to provide coordinated, participant-centered services.
