About The Position

The Case Manager coordinates care across an interdisciplinary team for a designated patient population. The role reviews inpatient admission records to assess appropriate use of hospital resources and whether admission is at the appropriate level of care. The Case Manager identifies discharge needs and coordinates an appropriate discharge plan, collaborates with the interdisciplinary team to support desired financial and quality outcomes, and maintains and enhances relationships with payors.

Requirements

Current BLS certification is required.
Three to five years of inpatient experience.

Nice To Haves

Licensure in the state where the hospital resides is preferred.
Inpatient experience in an acute, IRF, or LTACH setting is preferred.

Responsibilities

Coordinate interdisciplinary care for a designated patient population.
Review inpatient admission records to ensure appropriate utilization of hospital resources and determine whether the admission is at the appropriate level of care.
Assess and identify discharge needs and coordinate an appropriate discharge plan.
Collaborate with the interdisciplinary team to facilitate desired financial and quality outcomes.
Maintain and enhance payor relationships.
