About The Position

The Case Manager coordinates interdisciplinary care for a designated patient population. The role reviews inpatient admission records to evaluate utilization of hospital resources and whether admission is appropriate for the level of care. The Case Manager assesses discharge needs, coordinates an appropriate discharge plan, and works with the interdisciplinary team to support desired financial and quality outcomes. The role also maintains and enhances payor relationships.

Requirements

Current BLS certification is required.
Three to five years of inpatient experience.

Nice To Haves

State licensure where the hospital is located is preferred.
Inpatient experience in an acute, IRF, or LTACH setting is preferred.
Professional discipline may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
