About The Position

The Case Manager coordinates care across an interdisciplinary team for a designated patient population. The role reviews inpatient admission records to support appropriate use of hospital resources and determine whether patients are admitted at the appropriate level of care. The Case Manager assesses discharge needs, coordinates appropriate discharge plans, and collaborates with the interdisciplinary team to achieve desired financial and quality outcomes. The position also maintains and enhances payor relationships.

Requirements

Current BLS certification is required.
Three to five years of inpatient experience.

Nice To Haves

State licensure where the hospital is located is preferred.
Inpatient experience in an acute, IRF, or LTACH setting is preferred.
Professional discipline may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
