About The Position

The Case Manager coordinates care across disciplines for a designated patient population. The role reviews inpatient admission records to evaluate utilization of hospital resources and whether patients are admitted at the appropriate level of care. It also assesses discharge needs and coordinates suitable discharge plans, while working with the interdisciplinary team to support desired financial and quality outcomes and maintain and enhance payor relationships.

Requirements

Current BLS certification is required.
Three to five years of inpatient experience.

Nice To Haves

State licensure where the hospital resides is preferred.
Inpatient experience in an acute, IRF, or LTACH setting is preferred.
Professional discipline may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.

Responsibilities

Coordinate interdisciplinary care for a designated patient population.
Review inpatient admission records to evaluate hospital resource utilization and appropriate level of care.
Assess patients’ discharge needs and coordinate appropriate discharge plans.
Collaborate with the interdisciplinary team to achieve desired financial and quality outcomes.
Maintain and enhance payor relationships.
